Hospice Program

Home & Community Health Services' (H&CHS) Hospice Program provides health services and support for patients and their loved ones when curing a disease is not a reality. It is a special kind of care that supports the whole person - mentally, spiritually, emotionally and physically. The H&CHS hospice team treats the person, not the disease; focuses on the patient, as well as his/her family; and emphasizes the quality of life, not the duration.

An individual plan of care is developed for each patient and family. Directed by the patient's physician, medical care is aimed at relieving the patient's pain and other symptoms. The hospice nurse provides regular reports to the physician and coordinates other social, emotional and spiritual services, often involving pastoral and bereavement coordinators, home care aides, therapists, social workers and volunteers.
