"The hospice team we had was very professional, caring, informative, and prompt to respond to calls. We were very fortunate to have such caring individuals in the last days of my father's life journey."

Hospice Program

Home and Community

Home & Community Health Services, Inc. Hospice, an affiliate member of the Johnson Memorial Medical Center, provides choice, comfort, education and support for those with life limiting illnesses and those who care for them. By focusing care on comfort and striving to meet physical, emotional, social and spiritual needs, hospice seeks to assure that patients and their families have the highest quality of life possible. We offer specialized care that supports the whole person - mentally, spiritually, emotionally and physically.
The HCHS hospice team treats the person, not the disease; focuses on the patient, as well as his/her family; and emphasizes the quality of life, not the duration.

Home and Community

An individual plan of care is developed for each patient and family. Directed by the patient's physician, medical care is focused on end-of-life symptom management. The hospice nurse provides regular reports to the physician and coordinates other social, emotional and spiritual services, often involving pastoral and bereavement coordinators, home care aides, therapists, social workers and volunteers.
