Home & Community Health Services

Since 1908, Home & Community Health Services (H&CHS), formerly the Enfield Visiting Nurse Association, has provided home health and hospice care to residents of north central Connecticut and the bordering towns of Massachusetts. As an integral part of the Johnson Memorial Medical Center family, H&CHS is committed to providing care that maximizes the health, independence and quality of life enjoyed by residents of these communities.

Home healthcare services are appropriate for those coping with chronic illness, those recently hospitalized due to illness and those who require additional help to live safely at home. Whatever the need, H&CHS' skilled team of professionals delivers only the highest quality care, while also ensuring that patients and their families are treated with the utmost compassion, understanding and respect.

H&CHS is licensed by both the states of Connecticut and Massachusetts. The agency has recently been selected as one of the 2008 HomeCare Elite, the inaugural compilation of the most successful home care providers in the country.

The mission of H&CHS reaches beyond caring for patients in their homes. We are also committed to promoting a healthy community.
